- W2330590953 abstract "筋萎縮性側索硬化症の上位運動ニューロンと下位運動ニューロンにおける病変の程度と死亡時年齢の関係, 老年性変化と死亡時年齢の関係を, 55例 (40歳代8例, 50歳代8例, 60歳代20例, 70歳代12例, 80歳代7例, 罹病期間6カ月～14年) について検討した. なお, 各年代に正常5例を対照とした. 罹病期間と死亡時年齢には明らかな相関は認められなかったが, 各年代とも人工呼吸器使用例では罹病期間が延長し, 非使用例に比べると病変は強かった.病変と死亡時年齢の関係は頚髄膨大部前角に最も現れており, 若年者に比べると高齢者では明らかに病変が軽かった. 40歳代では前角神経細胞の著明な脱落, 線維性グリオーゼとともに前角の萎縮がみられた. 80歳代ではほとんど前角の萎縮は認められず, 神経細胞の脱落も軽度であったが, 残存細胞の多くはリポフスチン沈着が高度であった. グリオーゼも軽度であった. このような変化は腰髄, さらに脳幹運動核にも認められた. 一方錐体路に関しては, 若年者ではほとんどの症例に変性が認められたのに対して, 高齢者では変性が高度な症例とほとんど変性を見いだせない症例があるため, 前角病変ほど加齢との関係は明瞭ではなかった.一方, 大脳皮質を中心にした老年性変化は5例を除いて, 高齢者ほどアルツハイマー神経原線維変化および老人斑が出現していたが, 対照例と差が見いだせず, 本症で老年性変化が対照例に比べて加速されているという形態学的証拠はなかった. 5例はいずれも60歳以上で, 明らかに正常の上限を越える老人斑が新皮質に出現していたが, そのうち痴呆を認めたのは1例のみであった." @default.
