We sought to assess the rate of and predictors of clinically meaningful improvements (MIC) in LUTSBPH in patients treated with either silodosin as a monotherapy (SIL) or silodosin plus Ser (SIL+Ser) in the real-life setting. METHODS Complete data from the last 190 patients seeking medical help for LUTS/BPH at a single academic outpatient clinic and who have been treated with either SIL or SIL+Ser were collected. All patients completed the IPSS at baseline and after a minimum of 6 months of treatment. For the specific purpose of the study, only patients with baseline IPSS >12 were considered. Two MICs were assessed: MIC1 >3 points of improvement in terms of total IPSS; and, MIC2 >25% of improvement in terms of total IPSS from the baseline. Comorbidities were scored with the Charlson Comorbidity Index (CCI). Prostate volume (PV) and Qmax were collected at baseline. Descriptive statistics and logistic regression models were applied to assess rates of and predictors of MIC. RESULTS Of 190 patients, data from 97 (51.1%) SIL and 93 (48.9%) SIL+Ser patients were analysed, with a men (SD) follow-up of 10.7 (4.1) months. Overall, baseline total PSA, PV and Qmax were 2.8 (2.9) ng/mL, 54.6 (29.4) mL and 12.6 (6.6) mL/sec, respectively. Baseline IPSS and QoL scores were 17.6 (5.5) and 3.35 (1.5), respectively. No differences in terms of age, CCI, BMI, baseline IPSS, PV and Qmax were observed between groups. At mean follow-up assessment total IPSS score significantly decreased after both SIL (pre vs. post: 16.9 vs 14.5; p<0.001) and SIL+Ser (18.25 vs. 11.8; p<0.001). However, total IPSS was lower in patients treated with SIL+Ser than in those treated with SIL [14.5 (5.5) vs 11.8 (6.0), p=0.002]. A greater proportion of SIL patients still had IPSS scores suggestive for severe LUTS than SIL+Ser men (18.6% vs. 11.8%; p=0.01). MIC1 (59.1% vs. 40.9%; p<0.001) and MIC2 (64.0% vs. 36.0%; p<0.001) were achieved in a greater proportion of SIL+Ser patients compared to SIL patients. At MVA, age (OR 0.95; p=0.04) and baseline IPSS (OR 1.1; p=0.005) achieved independent predictor status for MIC1 after accounting for CCI, type of treatment, PV and Qmax. Similarly, age (OR 0.95; p=0.03), baseline IPSS (OR 1.1; p=0.04) and SIL+Ser treatment (OR 2.9; p=0.01) were independent predictors of MIC2. CONCLUSIONS The combination of SIL and Ser led to MIC in approximately 60% of LUTS/BPH patients.
