- W2330784759 abstract "In the past 20 years, prevention of gestational weight loss (GWL) has been an increasing priority for clinicians. However, recent observations have suggested that GWL is not always detrimental and could be beneficial for obese mothers with respect to several maternal and fetal outcomes. Several recent studies have reported that modest GWL among overweight and obese mothers does not result in adverse birth weight outcome for term deliveries. The aim of this retrospective cohort study was to identify potential beneficial or adverse associations of GWL with pregnancy-related outcomes and to determine whether beneficial associations offset the reported negative consequences of GWL. The study determined the risk of adverse pregnancy outcomes for GWL compared with nonexcessive gestational weight gain, adjusting for potential confounders and maternal body mass index (BMI) with stratification at booking for the following categories (kg/m2): underweight (BMI < 18.5), normal weight (18.5–24.9), overweight (25–29.9), obese class I (30–34.9), obese class II (35–39.9), and obese class III (≥40). Between 2000 and 2007, data were obtained for 709,575 singletons delivered in Bavarian obstetric units. Outcomes calculated for each BMI class included pregnancy complications (preeclampsia and nonelective cesarean delivery) and adverse neonatal birth outcomes (preterm delivery and the infant being born small-for-gestational age (SGA) or large-for-gestational age (LGA). GWL was associated with a lower risk for preeclampsia (adjusted odds ratio, 0.59; 95% confidence interval [CI], 0.39–0.91) among obese class II women and for nonelective cesarean (adjusted odds ratio, 0.65; 95% CI, 0.51–0.83) among obese class I women. In contrast, GWL was associated with significantly increased risks of preterm delivery and SGA births among overweight and obese class I/II mothers; the odds ratio for SGA births in obese class I women was 1.68 with a 95% CI of 1.37 to 2.06. No significantly increased risk of poor neonatal outcomes was found among obese class III women. These findings show that the association of GWL with reduced risk of pregnancy complications among overweight and obese mothers does not necessarily outweigh the increased risk of GWL for adverse neonatal outcomes in all BMI categories except obese class III mothers." @default.
