- W2331719005 abstract "直腸癌術後の縫合不全が再燃し難治性な症例に対して新しい内視鏡治療機器OTSC®（Over the Scope Clip）systemにより瘻孔を閉鎖しえた１例を経験した．70歳代男性，２年前に発症した縫合不全瘻孔部の閉鎖傾向が悪いため，内視鏡的・透視下にOTSC® systemにて瘻孔部の閉鎖を行った．瘻孔閉鎖翌日に38℃の発熱を認めたが一日で解熱，その後は炎症所見なく，骨盤CTでも膿瘍は消失した．留置後５カ月目の現在膿瘍腔の再燃は認めていない．直腸癌術後の縫合不全瘻孔部位は炎症のための線維化で肥厚化し従来の金属クリップでは閉鎖困難である．新しい瘻孔閉鎖用のOTSC®では内視鏡の口径以上の大きさのクリップを，瘻孔部に留置することが可能であり難治性の縫合不全部でも閉鎖が可能であった．2011年から保険適用され，今後の縫合不全の治療の新しい治療選択として有用である．" @default.
