- W2331799352 abstract "Periodic fever with aphthous stomatitis, pharyngitis and cervical adenitis(PFAPA)は，周期性発熱，アフタ性口内炎，頸部リンパ節炎，咽頭炎を主症状とし乳幼児期に発症する非遺伝性自己炎症性疾患である。溶連菌感染後急性糸球体腎炎(poststreptococcal acute glomerulonephritis; PSAGN)は一般的に保存療法のみで治癒する予後良好な疾患で，血液浄化療法を必要とする重症例はまれである。症例は7歳男児。平成23年2月，発熱・咽頭痛に対して急患診療所で抗生剤を投与され1日で解熱。約2週間後に発熱，嘔吐，乏尿，浮腫のため 総合病院へ入院となった。炎症反応陽性，低 Alb血症，高K血症，腎機能障害を認めたが， ASOの上昇はなく，急性胃腸炎，脱水症として補液を施行された。しかし高K血症と腎機能障害増悪，血尿・蛋白尿，低補体血症(C3 15 mg/dl)が判明し当科へ紹介入院となった。無尿，腎機能障害(BUN 84 mg/dl，Cre 2.93 mg/dl)のため，血液透析を10日間施行し尿量の増加とともに腎機能は改善した。抗 dsDNA抗体は陰性であり，臨床経過からも AGNが疑われた。腎生検では，典型的なびまん性管内増殖性糸球体腎炎像に加えて細胞侵潤を伴う軽度の間質性腎炎の所見が得られた。組織 nephritis-associated streptococcal plasmin receptor染色，plasmin活性染色陽性所見と併せてPSAGNと診断した。本症例の PSAGN重症化の背景にあるPFAPAの何らかの免疫調整異常が関与した可能性も否定できない。両者の合併により臨床経過が修飾された可能性もあり興味深い症例と考えられた。" @default.
