- W2332364653 abstract "Resume Les mediastinites postchirurgicales sont des infections graves, compliquant une chirurgie cardiaque ou œsophagienne. L'incidence des mediastinites apres sternotomie pour une chirurgie cardiaque est variable, de 0,15 % a 8 %. Les mediastinites postchirurgicales sont associees a un taux de mortalite extremement eleve, voisin de 50 %, principalement en raison du retard diagnostique et therapeutique initial. Un choc septique constitue la presentation clinique habituelle. La radiographie du thorax, le scanner thoracique, le transit œso-gastro-duodenal constituent des examens complementaires essentiels pour l'etablissement du diagnostic. Leur traitement repose sur une antibiotherapie synergique, une aspiration echoguidee ou un drainage chirurgical associe a la chirurgie plastique, le traitement precoce du choc septique et des defaillances viscerales associees." @default.
