- W2333131755 abstract "慢性閉塞性肺疾患（Chronic Obstructive Pulmonary Disease）罹患例では、骨折リスクが高まることが知られている。しかし、COPD症例において骨強度が低下するメカニズムは明らかではない。骨強度は骨密度と骨質によって規定される。COPD例では、骨密度の低下が報告されいるが、骨質の状況については明かでない。これまでに筆者らは、骨の単位体積当たり50％を占める骨コラーゲンの分子間架橋の異常が骨質低下の主たる要因であることを明らかにしてきた。コラーゲンの分子間架橋の内、老化化である終末糖化産物（AGEs）が骨強度を低下させることを種々の病態で見出した。そこで、COPD症例における骨脆弱化の機序を解明することを目的として、剖検例を用いたCase-Control研究を行った。COPD例、IP例、DM例と、これらの罹患の無い対照群から、骨、肺組織、気管軟骨、胸部大動脈、皮膚を採取し、総AGEs量を解析し、比較検討したところ、対照群に比べてCOPD例と糖尿病例では有意に高値であることが明らかとなった。これに対し、骨以外の組織のAGEs化は各群間に差はなかった。骨におけるAGEsの増加は、コラーゲン線維の強度変化に直接的に悪影響を及ぼすのみならず、骨芽細胞機能の低下や破骨細胞の活性化を誘導するという生物学的な側面でも悪影響を及ぼす。COPDではこれまでに報告されている骨密度の低下と共に、骨へのAGEsの増加という骨質の低下も併発することにより骨の脆弱性が高まると考えられる。" @default.
