It is a welcome text given the 21st century zeitgeist of the perioperative physician that extends the traditional role and responsibilities of the anaesthetist beyond the operating theatre. As clinicians we are increasingly responsible for the care of patients with complex problems and advanced age, receiving an ever-growing range of treatments and surgical interventions, in a variety of hospital settings. The book boasts an impressive and international group of contributors, with wide-ranging expert experience in all fields of perioperative medicine. With its new title, come several new chapters, and updates and revisions, reflecting new best-evidence. The book is divided into twenty eight chapters, providing an evidence-based summary of pre, peri and postoperative care. Chapters look at all elements of the high-risk patient’s journey through hospital. New chapters include obstructive sleep apnoea, high-risk neurotrauma and smoking, alcohol and recreational drug abuse. Weight is given to assessment of risk and perioperative optimization, in addition to intraoperative strategies to attenuate morbidity and postoperative complications. Each chapter is well referenced, with a robust evidence body. Particularly noteworthy publications are well represented, and those that have sparked controversy and debate. Many of the individual chapters focus on specific patient comorbid diseases, including transplant, obesity, and patients with cardiac disease and cancer. These chapters are especially useful for their pragmatic and evidence-based recommendations and will prove indispensible when searching for information that typically might only be found in more niche publications. The text avoids an overly didactic approach, instead presenting evidence for and against arguments, a style commonly overlooked in anaesthetic texts. A balanced debate on fluid therapy, cardiopulmonary exercise testing and enhanced recovery allow the reader to assimilate best new evidence described by experts in the field. The layout is appealing to the reader, with bullet points, tables and graphical figures to display information clearly and coherently. The chapters do not have a uniform framework. Some give greater emphasis to principles and pathophysiology, others favouring management and recent debate. The book is the richer for not adhering to a rigid framework, and the authors have clearly worked hard to provide stimulating and thought provoking reading. The book is particularly helpful in illustrating practical management and evidence-based concepts in a climate where appetite for perioperative team-based care is expanding rapidly. All healthcare workers who interface with the high-risk surgical patient will benefit from reading some, if not all sections of this book. Those with considerable experience will undoubtedly benefit from a refresher in recent best-evidence, whilst trainees and healthcare workers with less experience of perioperative care of the high-risk patient will welcome a problem-oriented introduction to new concepts. Much of the books content will be of immense help to the trainee revising for postgraduate examinations, and provides up-to date evidence-based knowledge that many textbooks lack. The authors and editor deserve praise for this updated edition that provides informed and balanced information reflecting modern day perioperative medicine. It is almost impossible to find fault with such a thorough yet straight-talking book, drawing together the themes common to the high-risk surgical patient. Closer mention of the challenges of radiology, transfer and the trend to minimally-invasive radiologically-guided procedures would be helpful. Given the collaborative input from both sides of the Atlantic, the editor should be congratulated on the use of simple language, avoidance of jargon terms and practices that remain extremely relevant and reflective of UK practice. Easy to digest, it promises to be a very welcome resource to departments and individuals alike." @default.
