- W2334302142 abstract "Background: Living donation is increasingly being carried out in the context of the secular trend of increasing prevalence of the metabolic syndrome (MS) worldwide. There is a paucity of data on the associations between metabolic syndrome at donation and 1) the presence of histologic abnormalities including acute tubular necrosis (ATN) among living kidney donors (LD) and 2) post donation kidney function Hypothesis: The presence of features of MS in the living donor would be associated with greater prevalence of histologic abnomalities and decreased renal function. Setting/Study Population/Methods: The sample for this restrospective analysis consisted of 459 living donors with implant biopsy (IMBx), of which 268 had at least 3 months of follow up and 142 had at least up to 2 years of follow up. MS was present in 65/459 (14.2%) donors. We studied associations between MS, kidney volume, kidney histology and renal function in a multivariate modelTable: [Donor characteristics by presence/absence of MS]Results: Donors with MS were more likely to be men and older; MS prevalence did not differ by race. By definition, donors with MS were more likely to be hypertensive and heavier. Masked hypertension (24-hour ABPM) was not different between groups. Donors with MS were also more likely to have elevated levels of total cholesterol (213±54 vs. 189±37 mg/dl, p< 0.01), LDL (130±47 vs. 110±34 mg/dl, p< 0.01) and uric acid (5.7±1.2 vs. 4.9±1.2, p< 0.01). Kidney volumes adjusted for body surface area were not different between groups (p=0.277), but donors with MS were more likely than donors without MS to have an abnormal histology (p=0.001). Various degrees of ATN likely stemming from the nephrectomy process were more likely in donors with MS than in healthier donors (p=0.049). While abnormal histology did not relate to kidney volumes, those with abnormal histology were more likely to have a lower GFR (104±15 vs. 108±16 ml/min/1.72 m2, p=0.035). There was also a statistically significant correlation between kidney volumes and GFR (p< 0.01). Conclusions: Histologic abnormalities in the donated kidney are more common among donors with MS and are associated with lower donor GFR.
