- W2334451278 abstract "The J-CTO scoring system (J-CTO) is a contemporary model predicting success rate in CTO PCI. Simple angiographic variables of blunt entry, calcification, lesion length >20mm & angle>45 degrees, and previous PCI attempt have been shown to be independent predictors of CTO PCI failure. While the use of bilateral injections and retrograde approaches during CTO PCI remains operator dependent, these contemporary strategies have been shown to improve success rates but may increase periprocedural complications. We evaluated the validity of the J-CTO scoring system at our standalone PCI centre and examined associations between the J-CTO score & CTO PCI approach. Lesion characteristics for J-CTO scoring of 172 lesions were collected from 170 patients following angiographic review. Data on the PCI procedure was collected from the medical records and the APPROACH database. The independent predictors of failure as per the J-CTO score were each allocated a point. Guidewire manipulation time was measured from the wire exiting the guide catheter until the wire crossing the lesion in successful attempts. Of the 172 lesions studied, PCI was successful in 132 lesions. The mean J-CTO score in the successful PCI cohort was lower when compared to the unsuccessful one (1.5 vs 2.0, p = 0.0063). Of the successful PCI group, the J-CTO score distributions of 0 to 4 were 10.5%, 33.2%, 34.2% 19.1% and 3.0% respectively. The average guidewire manipulation time before crossing was shorter in lesions with lowest J-CTO score(Figure 1) while success rates of crossing under 30 minutes was highest in lesions with the lowest J-CTO scores (Figure 2).
