- W2334451668 abstract "褐色細胞腫の遺伝子診断は，今世紀に入り大きく考え方が変わった分野である．その理由は，（1）近年のSDHBおよびSDHDの発見，（2）臨床的に散発性でも潜在的な遺伝性である可能性があること，（3）悪性化と関係する遺伝子（SDHB）が判明した事に集約される．遺伝性の頻度は，以前から知られていたVHL・RET・NF1に（1）（2）を加えると「10％ルール」で言うところの10％よりかなり高く25％程度と見積もられる．この中でSDHB・SDHDはTCA回路のコハク酸脱水素酵素サブユニットをコードする遺伝子であり，特にSDHB・SDHD変異による遺伝性褐色細胞腫・パラガングリオーマをHereditary pheochromocytoma/paraganglioma syndrome「HPPS」と呼ぶことが多い．SDHBの変異は腹部の副腎外褐色細胞腫（パラガングリオーマ）から発症し，その後高率に遠隔転移（悪性化）する．SDHDの変異は頭頸部の多発性パラガングリオーマを発症する（悪性化は少ない）．SDHB，SDHDについてはその遺伝子診断の臨床的有用性はまだ不明な部分も多いが，現在進行中である国内での多施設共同研究を通して，今後同検査が褐色細胞腫診断の標準的医療の一部と認知されるようにしたいと考える．" @default.
