- W2334552904 abstract "急性大動脈解離には様々な合併症が知られているが，術前の横紋筋融解症の合併は稀である．症例は69歳女性．意識を消失し，転倒しているところを発見された．胸部造影 CT にて血栓閉塞型急性 A 型大動脈解離（DeBakey II型）と診断された．入院時，奇異呼吸を伴う酸素化不良を認め，また胸背部痛とともに著明な両下肢痛を認めた．血液検査では筋原性酵素の上昇，急性腎機能障害の所見を認めたが，造影 CT 上，腹腔内臓器および下肢虚血の所見は認めなかった．補液など保存的治療にて筋原性酵素は徐々に低下し，急性腎機能障害も改善した．しかし経過中厳重な降圧管理にもかかわらず，偽腔増大，再解離の所見を認めたため上行大動脈人工血管置換術を施行した．術中施行した筋生検では，筋細胞の融解消失やリンパ球浸潤など筋原性変化を認めたが，各種自己抗体はすべて陰性であった．術前の筋原性酵素異常高値の原因として一過性ショックによる横紋筋融解症が最も考えられた．" @default.
