- W2334606219 abstract "The most recent rates of amputation for major extremity combat trauma are approximately 5.2% of all serious injuries and 7.4% of serious extremity trauma. In the acute setting, the patient’s overall physiologic condition determines how aggressively limb salvage should be pursued. Aggressive resuscitation is essential for combat-injured patients. Rapid hemorrhage control with tourniquets and hemostatic agents, high plasma ratio transfusion, and early correction of metabolic acidosis must occur prior to any prolonged attempts at limb salvage. Rapid stabilization of skeletal injuries and use of temporary vascular shunts can provisionally restore mechanical stability and limb perfusion while the patient is resuscitated. Once the patient is hemodynamically stabilized, vascular repair and aggressive serial debridement of the wound is completed while transporting the casualty to a higher level of care. Soft-tissue, nerve, and skeletal reconstruction should take place away from the battlefield and will determine the ultimate function of the injured limb. Currently, the only absolute indications for acute amputation on the battlefield are completion of a grossly contaminated traumatic amputation, the physiologically unstable patient with a mangled extremity, irreparable vascular injury, or an extremity with warm ischemia time greater than 6 hours." @default.
