L’hyperhomocystéinémie est actuellement reconnue comme un facteur de risque indépendant de thromboses artérielles et veineuses. L’objectif de notre étude était de déterminer la prévalence de l’hyperhomocystéinémie chez des patients atteints d’ulcères de jambes de cause vasculaire. Il s’agissait d’une étude descriptive, prospective, bicentrique d’évaluation d’une pratique réalisée entre mars 2008 et juin 2009 chez des patients hospitalisés pour des ulcères de jambes de cause vasculaire. Les antécédents cardiovasculaires étaient recueillis et le statut nutritionnel évalué à l’aide du score MNA. Le bilan biologique comportait un dosage standardisé de l’homocystéine, de la vitamine B12, des folates sériques, de la créatinine, de l’albumine, de la pré-albumine et un hémogramme. Soixante-huit patients étaient inclus dans l’étude dont 48 femmes et 20 hommes, d’âge moyen 77 ans. Cinquante-trois pour cent des patients avaient un ulcère veineux, 18 % un ulcère artériel et 29 % un ulcère mixte. La prévalence de l’hyperhomocystéinémie était de 56 %, sans différence significative selon le sexe ni selon le type d’ulcère. La prévalence de l’hyperhomocystéinémie dans notre population d’ulcères de jambes est élevée. Toutefois, cette étude descriptive ne permet pas d’établir de lien de causalité et l’absence de réduction du risque cardiovasculaire ou thromboembolique par la diminution de l’homocystéinémie rapportée dans la littérature n’incite pas à poursuivre les investigations concernant le lien entre hyperhomocystéinémie et ulcère de jambe. Homocysteine is a sulphur-containing amino acid derived from methionine. Hyperhomocysteinaemia is now recognised as an independent risk factor for occlusive arterial disease and thrombotic venous disease. The aim of this study was to determine the prevalence of hyperhomocysteinemia in patients with leg ulcers. We prospectively investigated hospitalised patients for vascular leg ulcers between March 2008 and June 2009 at two dermatology centres. We collected details of cardiovascular disease and determined nutritional status by means of the MNA score. Fasting blood samples were taken and analyzed for homocysteine, albumin, prealbumin, folic acid, vitamin B12, creatinine and a complete blood count. Sixty-eight patients were enrolled in the study: 48 women and 20 men. Fifty-three percent of patients had venous leg ulcers, 18% had arterial leg ulcers and 20% had leg ulcers of mixed origin. The prevalence of hyperhomocysteinemia was 56%, with no differences according to ulcer type or gender. While the prevalence of hyperhomocysteinemia in our population of leg ulcer patients was high, this descriptive study does not allow us to establish any causal link between hyperhomocysteinemia and leg ulcers. Moreover, since the literature indicates that homocysteine-lowering therapy does not reduce cardiovascular and thromboembolic risk, there appears to be little call for further trials on hyperhomocysteinaemia and leg ulcers." @default.
