- W2336359617 abstract "With the current development in echocardiographic technologies, patent foramen ovale (PFO) can be identified in nearly 25% of the population. When a PFO is suspected, transthoracic echocardiography with contrast test using a Valsalva manoeuvre is the first step of examination. Transesophageal echocardiography is complementary in order to detect other abnormalities, especially an atrial septal aneurysm (ASA). Severe hypoxemia due to a large PFO is a well-recognized indication. Most often, PFO with or without ASA is diagnosed in a young patient after a first cryptogenic stroke. Indication of PFO closure must be discussed after a thorough examination and with a close collaboration with the neurologic staff. Randomized trials are necessary before the completion of guidelines. CLOSURE I study did not achieve to demonstrate the superiority of PFO closure in comparison with medical therapy. CLOSE study is an ongoing French multicenter study. Its goal is to randomize 900 patients between aspirin, anticoagulant or percutaneous closure. Others indications of PFO closure are discussed : migraines or decompression illness in divers." @default.
