- W2338329375 abstract "Introduction: Early laparoscopic cholecystectomy (ELC) is the gold standard treatment for acute cholecystitis (AC). However, optimal timing of cholecystectomy in elderly patients with significant comorbidities remains unclear. Is medical treatment a valid initial approach to AC, namely for patients >65y with comorbidities? Methods: Retrospective longitudinal analysis of 359 patients admitted to our Emergency Room with clinical and radiologic diagnosis of AC in 2013–2014. Tokyo III AC patients excluded. Two groups compared: A) medical treatment (n = 120) and B) early cholecystectomy (n = 239). Complications in A) defined as need of urgent cholecystectomy and early readmission due to AC persistence or complication. Complications in B) defined as Clavien-Dindo≥III. Statistical significance for alpha < 0,05. Results: A) patients were older (68,1±17,6 vs 58,7±17,5;T = 4,77 p < 0,001). Gender equally distributed. Cardiovascular and metabolic comorbidities higher in A) (χ2 = 16,36; p < 0,001). Length of stay and complications higher in A) (11,6±15,3 days vs 5,7±9,5 days;T = 3,84 p < 0.01| χ2 = 2 7,27 p < 0,01). No significant difference in mortality observed. Considering patients >65y and with ≥3 cardiovascular and metabolic comorbidities, length of stay and complications were higher in A). No significant difference observed for mortality. In logistic regression, medical treatment (OR = 4,41; 95%CI:2,41–8,08) and ≥3 cardiovascular and metabolic comorbidities (OR = 3,09; 95%CI:1,44–6,60) were risk factors for complications but not for mortality. These risk factors persisted for patients >65y, with a higher OR (5,94 95%CI:2,35–14,99 and 3,11; 95%CI 1,13–8,56 respectively). Conclusion: Medical treatment is not a valid alternative to ELC in acute cholecystitis given its higher morbidity, even in patients older than 65 with associated comorbidities." @default.
