Indeed, on both ethical and pragmatic grounds, it seems fundamental that children should be involved in medical decisions that affect their own health, 2,3 and their participation might contribute towards an improved outcome. 4 Nevertheless, the capacity of children to participate in a decisionmaking process in paediatric research has been questioned, 5 and several potential difficulties 3,6 that undermine the effectiveness of this type of decision-making need to be investigated more fully. 7 Consequently, various models of progressive involvement of children in medical decisionmaking have been proposed. 3,8 In view of these issues, what involvement should children have in such decision-making? There is no general answer to this question because of the underlying differences in each individual situation. The level of involvement of a child in a medical decision should be carefully tailored according to several variables. These variables include: the child’s age and competence; the child’s desire to participate; the consequences of the decision; the complexity and emergency of the medical situation; possible medical alternatives; the parents’ opinion; and the family’s perspective on the problem. 2–8 Therefore, children cannot automatically be the main stakeholder in the decision process. Instead, to ensure good patient-centred paediatric care, it would seem more appropriate to always aim for involvement of the child, but not to delay treatment if the child’s involvement cannot be obtained. Furthermore, this approach should be applied not only to key medical decisions, such as participation in randomised studies, but also to daily medical decisions and care.
