- W2340409780 abstract "Lyme disease is caused by the spirochete Borrelia burgdorferi, transmitted by Ixodes ticks icinus complex. In Mexico, the prevalence of 7.28% in the DF and 15% in the Northeast. Clinical case: Female, 29 years, booking agent for air travel. Current Condition: starts with pain in the posterior chest level right scapula, stitching type, without radiation, without other symptoms. On the third day of development, it added headache, increased pain intensity in the same region, asymmetric migratory arthralgias predominantly in the hands and knees, shivering. Physical examination: BP: 110/70, HR: 80, FR: 14, Temperature: 37 ∞, height: 163cm, weight: 68 kg. Alert, oriented, anxious, hydrated, with conjunctival hyperemia + + bilaterally, with appropriate color of skin, full, mesomorphic, without lymph node enlargement, with pupils normal. In the thorax: right subscapular region: 38 mm erythematous lesion with defined edges, without pain or itching. Tips: Joints with «type stitching pain» to bending, with swelling and limitation in range of motion. Is made markers of inflammation and rheumatic Profile: SGA: 8 mm / hr, ESR 6 mm / hr, CRP 33 mg / l, Anti-CCP negative, Anti DNA: negative, ANA: nuclear fine speckled 1:160 dilution, Anti Sm: negative, C3 88 mg / dl, C4 20 mg / dl and ANCA negative. Ultrasound scan was performed on data search and para-articular inflammation of joints reported: right peroneal tenosynovitis, carpal ganglion law and without ultrasound evidence of erosive arthropathy. Aspiration of the knee: 15 000 cells, 50% PMN and negative Gram stain and culture of synovial fluid negative. In the chest radiograph, hands, knees and feet are not observed alterations. The persistence of clinical symptoms despite treatment with NSAIDs, it was decided to take a biopsy of the lesion in search of Borrelia. The sample is sent to CMN century. Reported positive PCR for Borrelia burgdorferi. ELISA is requested Lab United States and is reported as positive for borrelia, thus confirming the diagnosis of Lyme disease." @default.
