- W2342580186 abstract "Aims: A tissue-sparing approach to pancreatic neoplasms, pancreatic enucleation is considered to result in better postoperative pancreatic function than standard pancreatic resection. The objective of this review was to compare the postoperative outcome after pancreatic enucleation vs. resection. Methods: This systematic review was conducted according to the recommendations of the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. MEDLINE, Embase and the Cochrane Library were searched systematically until December 2014 to identify studies comparing the outcome of enucleation vs. standard resection. After critical appraisal, meta-analyses were performed and the findings were presented as odds ratios or weighted mean differences with corresponding 95% confidence intervals. Results: Twenty-two observational studies (1148 patients) were included. Operation time, blood loss, length of hospital stay, and postoperative endocrine and exocrine insufficiency were significantly lower after enucleation. Mortality, overall complications, reoperation rate, and delayed gastric emptying did not show significant differences. The overall postoperative pancreatic fistula (POPF) rate was significantly higher after enucleation than after standard resection. However, the elevated POPF rate was not manifested clinically in terms of higher mortality or overall morbidity. A sensitivity analysis of high-volume studies (> 20 enucleations and > 4/year respectively) showed that, in specialised centres, enucleation can be performed without an elevated risk of POPF. Conclusions: Compared to standard resection, pancreatic enucleation can be performed effectively and with comparable safety in high-volume institutions. Enucleation should be preferred to standard resection for selected pancreatic neoplasms whenever possible." @default.
