À j2, la patiente présente un syndrome sérotoninergique sévère avec état de choc. Des dosages de la venlafaxine et de l’O-desméthylvenlafaxine ont été réalisés par LC-MS/MS et retrouvent respectivement dans le sang à j4 5842 et 386 ng/mL, à j11 2118 et 275 ng/mL, et dans les urines à j11 30 000 et 51 650 ng/mL, suggérant un défaut de métabolisation de la venlafaxine. Une analyse génétique réalisée chez cette patiente a permis de confirmer un polymorphisme du CYP2D6 à l’origine d’une perturbation du métabolisme de la venlafaxine. Approximativement 7 % des sujets de type caucasien présentent un génotype correspondant à une enzyme non fonctionnelle du CYP2D6, et présentent un phénotype de métaboliseurs lents. Ces sujets sont de ce fait susceptibles de présenter des intoxications particulièrement graves, ce qui était le cas de notre patiente. Dans ce contexte un syndrome sérotoninergique doit être soigneusement recherché car les signes sont aspécifiques et trompeurs et peuvent par ailleurs être masqués et/ou aggravés par la prise concomitante de médicaments neuroleptiques dont le tableau clinique est ressemblant. The antidepressant venlafaxine is a reuptake inhibitor of serotonin and norepinephrine. In case of poisoning, venlafaxine can elicit a severe serotonin syndrome with shock. Venlafaxine undergoes a strong hepatic metabolism by CYP2D6 to its major active metabolite, O-desmethylvenlafaxine. Here we report the case of a self poisoning of a 41 years old Caucasian woman, who ingested 1,200 mg of alimemazine, 11,700 mg of oxazepam and 13,500 mg of venlafaxine. On day 2, she presented a severe serotonin syndrome with cardiac shock. Venlafaxine and O-desmethylvenlafaxine dosage were carried out by LC-MS/MS and respectively found in blood at 5,842 and 386 ng/mL on day 4, and 2,118 and 275 ng/mL on day 11; and 30,000 and 51,650 ng/mL at day 11 in urine. Those results suggested a decrease in venlafaxine metabolism by our patient, that was thereafter confirmed by a genotyping which highlighted a polymorphism of CYP2D6 causing a disturbance in the metabolism of this drug. Approximately 7 % of Caucasians have a genotype with a non-functional CYP2D6 enzyme and present a phenotype of “poor metabolizers”. Those subpopulations are likely to develop serious intoxications, which was the case of our patient. Therefore serotonin syndrome must be carefully searched because the signs are non-specific and misleading. It can also be masked and/or aggravated by the concomitant use of neuroleptic drugs whose clinical expressions are similar." @default.
