- W2348751854 abstract "Despite the advances in immunosuppression and blockade of the renin-angiotensin-aldosterone cascade,the refractory nephrotic syndrome remains to be a therapeutic challenge.Adrenocorticotropin(ACTH),a pituitary neuroimmunoendocine polypeptide,was widely used about half century ago as an effective therapy for childhood nephrotic syndrome but has since largely been replaced by less costly,orally administered synthetic glucocorticoids.In addition to controlling steroidogenesis as a component of the hypothalamic-pituitary-adrenal axis,ACTH also acts as an important physiological agonist of the melanocortin system.Lately,accumulating clinical and experimental evidence suggests that ACTH possesses anti-proteinuric,lipid lowering and reno-protective activities.ACTH therapy is prominently effective in inducing remission of nephrotic syndrome in patients with a variety of proteinuric nephropathies,even those resistant to steroids and other immunosuppressants.This review describes the biology and physiology of ACTH,with particular emphasis on melanocortin effects.Findings from the latest clinical trials are also discussed with perspectives on potential applications of ACTH as a treatment for nephrotic glomerulopathies." @default.
