- W2349959738 abstract "Objective To compare the efficacy of patient-controlled epidural analgesia(PCEA)with patient-controlled intravenous analgesia (PCIA) for postoperative pain management in patients undergoing total hip arthroplasty.Methods Forty-eight patients,ASA status I～Ⅲ,scheduled for total hip arthroplasty were randomly divided into group E or group I. All patients had epidural anesthesia during the operation. Postoperatively,patients received an infusion of 1.25 mg/ml epidural ropivacaine with basal infusion of 3 ml/h,bolus dose of 5ml and lockout time of 30 min (group E),or 10 μg/ml intravenous fentanyl with basal infusion of 2 ml/h,bolus dose of 3ml and lockout time of 15 min (group I) using a patient-controlled analgesia pump for pain relief during 48 h. Pain scores (VAS),BP,nausea,vomiting,deliration,and puritus were recorded after surgery.Results Pain scores were less than 3 in both groups during 48 h postoperatively. At the first 12 h,PCEA provided lower VAS than did PCIA (P0.05),and VAS was comparable between groups during 24～48 h postoperatively. Patients with PCEA were more likely to experience hypotension,numbness of lower limb,and loss of epidural catheter (P0.05). Nausea,vomiting,deliration,and puritus were similar irrespective of pain management type.Conclusion PCEA may result in a better pain relief,but more difficult analgesia management when compared with PCIA after total hip arthroplasty." @default.
