- W2351898888 abstract "Objective:The scarcity of cadaver organ donation has forced the use of living donors still further.Strategies to increase living kidney donor transplantation are urgently needed. Methodology:All(n=74) living kidney transplantation were investigated at our hospital between January 2006 to December 2006 to determine the outcomes of elder donor transplantation.The elder donor was defined as 55 years and older.All of the 148 patients(donors and recipients) were divided into 2 groups based on donor age(55 or ≥55 years).All living kidney donors and recipients underwent 12-month follow-up at several endpoints postoperatively.Early posttransplantation outcomes,1 year graft and patient survival,renal function 1 year posttransplantation were compared between the two groups. Results:Recipients of age,gender,blood relationship,original renal disease and dialysis period pre-transplant had no odds of elder living donor transplantation.The elder living donor transplantation was most likely from parental donors in our research(10 of the 11 elder donors were parental donors).Early posttransplantation outcomes were excellent in recipients from elder donors.The levels of postoperative mean serum creatinine(SCr) at 1 week,2 week,and 1 year,and 3 years were(117±17.0),(101±15.5),and(112±16.8)μmol/L,respectively [compared to recipients from younger donors,(112±17.1),(102±14.3),(114±15.5)μmol/L,respectively].There were no significant differences between the two groups at the three endpoints(P0.05,0.05 and 0.05).In elder donors,the levels of mean Scr before the operation,1 week and 1 year postoperation were(85.6±13.7),(106±15.6),(88.0±11.4) μmol/L,respectively [compared to younger donors,(73.7±6.8),(102±13.1),(79.1±6.11)μmol/L,respectively].There were no significant differences in both of the groups between preoperative and 1-year postoperative SCr(P0.05,0.05,respectively).Their blood pressure,urine protein excretion and postoperative complications after donation had no differences in both of the groups. Conclusion:Our findings demonstrate that outcomes are excellent with utilization of elder living donors up to the age of 65 years old.The elder donor kidneys can provide good allograft function in most recipients.Furthermore,for the donors,the risks after donation are low." @default.
