Aim-In a pointprevalence studythe occurrence ofsensitive indices indicative ofearly kidneymalfunction wasassessed inoutpatients withinflammatory bowel disease. Methods-Routine indicesof kidney function (creatinine clearance, urinary proteincontent, pH, electrolytes, and microscopy) were investigated in 223 patients withinflammatory boweldisease aswellassensitive markersofglomerular ortubular dysfunction (microproteinuria bySDS polyacrylamide gelelectrophoresis(SDS-PAGE),urinaryconcentrationsofN-acetyl-1-D-glucosamidase, aol-microglobulin, -y-glutamyltransferase (GGT),alkaline phosphatase (AP),and albumin). Histories ofexposure to5-ASA wereassessed byquestionnaire. Results-Patients receiving highamounts of5-ASA,bothactualaswellason a lifetime basis,showed an increased prevalence oftubular proteinuria bySDSPAGE.Raisedvalues forurinary AP and GGT indicate proximal tubular epithelial cellsas thesource.Allotherkidney function testswerenormal.Analysis of covariates indicated strongassociations between disease activity andsizeof5-ASA dosesas wellas alterations inkidney tubular function. Conclusion-The possibility exists that highdosesof5-ASAmay beassociated withproximal tubular proteinuria.
