- W2356349861 abstract "[Objective]To investigate the advantages and disadvantages of 3 different treatments for lower lumbar vertebrae diseases,i.e.,unilateral translaminar facet screw fixation,unilateral pedicle screw combined with contralateral translaminar facet screw fixation,and bilateral pedicle screw fixation after posterior approach decompression and intervertebral cage fusion.[Methods]This study assessed a total of 84 cases of lower lumbar vertebrae disease in subjects who underwent one of 3 different surgeries:26 patients were treated using unilateral translaminar facet screw fixation by gunsight guiding,30 patients were treated using aunilateral pedicle screw combined with contralateral translaminar facet screw fixation by gunsight guiding,and 28 patients were treated using bilateral pedicle screw fixation.Data were collected to compare the operation time,length of the incision,blood loss duringthe operation,transported lymph after the operation,and the cost of these 3 surgical treatments.We also compared the intervertebral space,loosening of the fixation and intervertebral cage fusion,and the symphysic condition of intervertebral fusion pre-and post-operatively using imaging studies.The clinical effects were assessed and compared using the JOA Waist pain rating system.[Results]There were statistically significant differences in incision length,operative time,intraoperative bleeding,and postoperative transported lymph for the 3 surgeries.Among these 4 categories,group A was noted to be superior to groups B and C,while group B was superior to group C.All cases were followed up for an average of 20 months(range:12-36 months).In all 3 treatment groups,the stability of the intervertebral spaces of the lesions improved;however,follow-up data were missing for group A.The fusion rates noted in the follow-up data were 96.2%,96.7%,and 92.9% respectively.There were no statistically significant differences among these 3 groups.The JOA scores in the follow-up data were also statistically insignificant(P0.05).[Conclusion]Comparing bilateral pedicle screw fixation,percutaneous unilateral translaminar facet screw fixation,and unilateral pedicle screw combined with contralateral translaminar facet screw fixation have much advantages,including a short incision,minimal trauma,and a relatively simple operation.More specifically,the screw placement significantly improves accuracy,safety and decreases trauma.Moreover,as unilateral pedicle screw combined with contralateral translaminar facet screw fixation offers greater stability than unilateral translaminar facet screw fixation alone,it is a better choice for the treatment of lower lumbar vertebrae diseases." @default.
