- W2359197556 abstract "【Objective】 To investigate the value of TNM staging system combined with model for end-stage liver disease(MELD) score,the TNM-MELD score,for predicting survival in unresectable primary liver cancer(PHC) patients treated with trans-catheter arterial chemoembolisation(TACE).【Methods】 A total of 163 patients with PHC were enrolled into the study from January 2005 to October 2009.In the TNM-MELD scoring system,TNM stage Ⅰ,stage Ⅱ,stage Ⅲ,and stage Ⅳ,and MELD cut-off scores of ≤6,6～10,10～15,and ≥15,were allocated scores of 0,1,2,and 3,respectively.The summation of the tumor staging score and the MELD cut-off score were defined as the TNM-MELD score.TNM-MELD score was calculated for each patient following the original formula on the first day of admission.All patients were followed up at least 3 months.The predictive prognosis related with the TNM-MELD scoring model was determined by the area under the receiver operating characteristic curve(AUC),compared with TNM staging system and clinical diagnosis criteria issued by the Chinese Liver Cancer Association(CDC).Kaplan-Meier survival curves were made using the cut-offs identified by means of receiver operating characteristic curve(ROC).【Results】 At 3 month of enrollment,TNM-MELD score had the highest AUC(0.650),and was followed by TNM(0.641) and CDC(0.603).At 6 month of enrollment,TNM-MELD had the highest AUC(0.598),and was followed by CDC(0.594) and TNM(0.586).There was no significant difference between the three prognostic models.TNM-MELD score succeeded to differentiate between TNM-MELD score 4-patient populations versus score 1-(χ2=24.296,P =0.000),score 2-(χ2=8.653,P =0.003),and versus score 3-patient populations(χ2=4.598,P =0.032),and between TNM-MELD score 1-patient populations versus score 2-(χ2=9.489,P =0.002)and versus score 3-patient populations(χ2=10.461,P =0.001).【Conclusions】 TNM-MELD score can exactly predict the prognosis of unresectable PHC patients treated with TACE for short period." @default.
