- W2361446327 abstract "Objective: To investigate the feasibility of reducing the number of segments and Monitor Units(MUs) for IMRT plans of esophageal carcinoma.Methods:(1) Optimal beam angle: according to the target,non-uniform beam angles are given by experienced physicists,but conventional uniform beam angles.(2) The choice of intensity level: intensity levels are divided into four levels: 12,10,8 and 6,choose the appropriate level.(3) Concentric ring structures about the target: six concentric rings are continually defined around the PTV target while the five center ring are developed by 1cm width,the external ring is developed from the patient surface to the borders of the fifth ring,and dose equivalent limit is given for each ring.(4) Use direct machine parameter optimization(DMPO)for IMRT planning: in this mode,the segments number can be pre-limited,the shape and weighting of each segments can be optimized directly and the sequence of segments are determined directly.Results:(1)Comparing with conventional uniform beam angles,non-uniform beam angles which are selected by the target,receive better target dose conformity and uniformity,meanwhile,the mean exposure dose of lung and the max exposure dose of spinal cord are lower,but not for heart V50.For this method the total numbers of segments and MUs have no differences.(2)For the overall quality of the three plans which differ in intensity level(8,10,12) are very basic in qualities,but when the intensity level is decreased to 8,the number of segments can be reduced by 17%.(3)Defining concentric ring structure of the target and restriction the limitation dose of each ring,no matter the uniform of target dose,or the normal tissue dose are superior to non-concentric ring struc tures obviously.At the same time defining concentric ring structure of the target can reduce the number of segments by 23% and MUs by 9%.(4)Comparing with traditional intensity-modulated planning,DMPO achieves better conformity,meanwhile Dmean of lung、Dmax of spinal cord are lower,but the heart-Dmean.In total,using DMPO can achieve better quality in treatment planning.At the same time direct optimization with machine parameters can reduce the number of segments by 37% and MUs by 16%.Conclusions: The number of segments and Monitor Units(MUs) can be reduced by the choice of intensity level,defining concentric ring structure of the target and using direct optimization with machine parameters while the premise of the plans quality is not changed or even improved." @default.
