- W2362764409 abstract "Objective To evaluate the efficacy and safety of dural puncture epidural (DPE) technique on labor analgesia.Methods One hundred and eight nulliparous parturients were randomly allocated into 3 groups. Group P received patient controlled epidural analgesia (PCEA) with a loading dose of 5 ml 0.125% ropivacaine plus sufentanil 0.4 μg/ml followed by PCEA (background infusion 7 ml/h, bolus 2 ml, lock-out time 15 min). Group S received combined spinal epidural analgesia (CSEA), sufentanil 3 μg was intrathecaled, followed by PCEA as group P. Group D receive a single dural puncture with CSE needle, after the successful puncture and placement of epidural catheter, 0.062 5% ropivacaine 10 ml plus sufentanil 0.2 μg/ml was administered through the epidural catheter and a PCEA (background infusion 14 ml/h, bolus 4 ml, lock-out time 15 min) was initiated. Pain was assessed by VAS at 5 time points: before analgesia(T0), 1 min(T1),5 mins(T2),10 mins(T3) and 60 mins (T4) after analgesia . Consumption of sufentanil, PCA times, progress of labor and maternal side effects were recorded.Results At T2, the VAS of group S and group D were superior than that of group P(P0.05); at T4, the VAS of group D was superior than those of the other two groups (P0.05). Compared with group P, group D was detected a faster onset time, a longer interval between the loading dose and the first bolus, a less consumption of sufentanil, a shorter progress of labor and a less percentage of instrumental delivery. Compared with group S,group D was observed a less frequency of analgesia side effects, especially fetal heart rate deceleration and maternal fever.Conclusion With better effect and less side effects, DPE provide satisfied and safe labor analgesia." @default.
