- W2364192616 abstract "Objective To identify the asymptomatic coronary artery disease(CAD) in patients with type 2 diabetes mellitus(DM) or impaired glucose regulation(IGR) by using the 64-slice spiral CT,and to explore the indications and value for 64-slice coronary computed tomography angioplasty(CCTA).Methods One hundred and forty-nine asymptomatic patients(65 with DM and 84 with IGR) registered to Endocrinology Department of Shanxi Provincial People′s Hospital during August 2009 and March 2010,were selected in the study based on the inclusion criteria.All subjects underwent general physical examination,collection of diabetes related history,tests for blood glucose,blood lipids and blood pressure,and CCTA.37 patients received selective coronary angiography within 3 months after CCTA.Vessel occlusion ≥ 50% of the lumen was considered as significant stenosis.Results The sensitivity,specificity,positive and negative predictive values and accuracy rate of CCTA for detection of significant stenosis in DM and IGR patients complicated with asymptomatic CAD were 92.4 %,94.2 %,91.5 %,94.8% and 93.2% respectively,when comparing with CAG.Significant coronary stenosis was detected in 37(24.8%) from all the 149 patients enrolled,among whom,26(70.3%) were with single branch lesion,and 11(29.7%) with multiple branch lesions.Incidence of significant stenosis in DM patients was 41.5%,with 1/3 out of the patients detected to had lesions in at least two branches.11.9% of IGR patients were found to have significant stenosis,with 40% necessitating percutaneous coronary intervention(PCI).Patients with DM showed significantly higher prevalence of coronary stenosis and calcificaion plaques than patients with IGR.Moreover,there were more DM patients with multiple branch lesions that necessitated PCI compared with IGR patients.However,no statistical difference in detection of soft plaques was noted between the DM and IGR groups.Conclusion As a non-invasive examination,64-slice spiral CT is rather accurate in identifying asymptomatic CAD in DM and IGR patients,and is therefore important in non-invasive evaluation of coronary artery lesions.Screening with CCTA is indicated not only for DM patients,but also for IGR patients who have at least one risk factor for CAD." @default.
