• W2367749796 abstract "Objective To compare the therapeutic effects for nasal bi-level positive airway pressure ( BiPAP) and nasal continuous positive airway pressure ( NCPAP) combined with pulmonary surfactant in preterm infants with respiratory distress syndrome ( RDS) . Methods Eighty RDS patients were randomized to BiPAP group and NCPAP group with 40 cases in each group after using pulmonary surfactant. The ventilator parameters,oxygenation status,duration of oxygen dependence and ventilation administration,the success rate of treament and the incidence of different complications were observed between the two groups. Results ( 1) The ventilator parameters ( FiO 2 ,PEEP/EPAP) decreased with the time of ventilation for two groups. FiO 2 ,PEEP/EPAP at 0 h between the BiPAP and the NCPAPgroups were not statistically different ( P 0. 05) ,but they were lower in BiPAP group than in NCPAP group at 6 h,12 h,24 h,48 h after ventilation( P 0. 05 or P 0. 01) . ( 2) For two groups,there were both significant increase in oxygenation status ( PaO 2 ,P/F,PaO 2 / PAO 2 ) and decrease in PCO 2 . There were no difference of PaO 2 ,P/F,PaO 2 / PAO 2 at 0 h between two groups ( P 0. 05) ,but in BiPAP group there were higher in those NCPAP group at 6 h,12 h,24 h,48 h after ventilation ( P 0. 05,P 0. 01) ; while PaCO 2 was lower than that of control group at the same time ( P 0. 05 ) . ( 3 ) The duration of oxygen dependence and ventilation for the BiPAP group were shorter than the NCPAP group ( P 0. 05) ,whereas the success rate in BiPAP group was higher in NCPAP group ( P 0. 05) . ( 4) No significant difference in the incidence for pneumothorax, bronchopulmonary dysplasia, premature retinopathy were found between two group ( P 0. 05 ) . Conclusions Combined with pulmonary surfactant,the effect of nasal BiPAP appears to have greater advantage over that of NCPAP obviously for preterm infants with RDS. It has a value of high efficiency and safety,may be worthy of the widely application." @default.
