- W2368162512 abstract "AIM To probe into the feasibility of transcatheter closure of atrial septal defect (ASD) with Amplatzer septal occluder (ASO) device directly guided by transthoracic echocardiography (TTE ) measured diameter. METHODS There were 84 patients. The mean age was 23.6±11.4 years (ranging from 1 to 64 years). The patients were divided into 2 groups: Balloon group (group A) and non balloon group (group B) according to the guided used in ASO selection. There were 36 patients whose ASDs were occluded with ASO selected based on balloon stretched diameter (BSD) in balloon group . There were 36 patients in non balloon group .TEE measured diameters were used for selection of ASO size. RESULTS There was no significant difference between the two groups in the largest ASD diameter measured by TTE [(18.6±7.5) mm vs (19.1±7.2 mm)], size of ASD edge and the size of implanted ASO [(21.4±8.6) mm vs (12.6±8.2 mm)] devices ( P 0.05). Both procedural time and fluoroscopy time in group A were longer significantly than those in group B [(64.4±13.8) min vs (43.7±10.4) min]; [(13.1±6.3) min vs (8.1±5.2) min, P 0.01, respectively]. The occlusion were successfully achieved with TTE in 82 (97.6%) patients, but 2 cases still needed help of ASD TEE. The rates of complete closure of ASD were not significantly different between the two groups soon after implantation, 24 hours after implantation, and in a short time follow up duration ( P 0.05). Correlation between the largest diameter of ASD measured by TTE and BSD were fine( r=0.957, P0.01 ) in group A. The largest diameter of ASD correlated well with the ASO selected ( Y=1.03X+ 4.45, r=0.946, P0.01) . CONCLUSION Transcatheter closure of secundum ASD occlusion with ASO selected directly on the basis of TTE measured diameter is safe and efficient in ASD patients . Large and short edge ASD or ASD with thin edge and difficulty of detection by TTE should be monitored by TEE before the procedure, and could be done by TTE or TEE." @default.
