- W2369008829 abstract "[Objective]We explored the relative positional relationships between the fracture fragments of the posterior malleolus and achilles tendons to provide a reference for decisions regarding posterior malleolus surgical approaches.[Method]We identified the malleolar multilayer spiral computed tomography(CT) imaging data of patients with posterior malleolus fracture treated in the 101stHospital of the Chinese People's Liberation Army from March 2010 to May 2013,and then reconstructed the fracture fragments of the posterior malleolus and the rear achilles tendons using three- dimensional(3D)- reconstruction techniques.Based on these 3D- reconstruction images,we observed the relative positions of the fracture fragments of the posterior malleolus and rear achilles tendons.Then,we identified the CT scanning images that clearly showed the main fracture fragments of the posterior malleolus projected from the posterior malleolus to the front edges of the achilles tendons,chose the midpoints of the front edge of the achilles tendons as the reference points,and observed the characteristics of the fracture fragment distribution in the posterior malleolus.[Result]Three relative positions were observed between the fracture fragments of the posterior malleolus and rear achilles tendons:1) In 33 cases(57.9%),the projections of the posterior malleolus fracture fragments to the front edges of the achilles tendons were limited to the lateral edges of the achilles tendons and extended beyond the medial edges of the achilles tendons;2) In 2 cases(3.5%),the projections of the posterior malleolus fracture fragments to the front edges of the achilles tendons were limited to the medial edges of the achilles tendons and extended beyond the lateral edges of the achilles tendons;and 3) In 22 cases(38.6%),the projections of the posterior malleolus fracture fragments to the front edges of the achilles tendons extended beyond both the lateral and medial edges of the achilles tendons.In the CT images that clearly showed the main fracture fragments of the posterior malleolus,we observed that in 55 of 57 cases,the projections extended toward to the medial edge,whereas in 24 of 57 cases,the projections extended toward the lateral edge.We also found that the mean length of the projections to the medial edge was 23.04 ± 8.24 mm and that to the lateral edge was 6.82 ± 4.63 mm;this difference was statistically significant(P 0.05).[Conclusion]Observing the relative positions of the fracture fragments of the posterior malleolus and achilles tendons in a posterior malleolus fracture and the characteristics of the fracture fragment distribution are necessary,and will aid in better exposing the posterior 078malleolus fracture fragments during surgery.Malleolar multilayer spiral CT scans and 3D- reconstruction of the fracture fragments of the posterior malleolus and achilles tendons are required before surgery,and the choice of the approach to the posterior malleolus should be decided after achieving a comprehensive understanding of the relative positional relationships between the fracture fragments of the posterior malleolus and achilles tendons." @default.
