- W2370251744 abstract "Objective To investigate the condition of impaired glucose metabolism in patients with adrenocortical adenoma,and its correlation with high secretion of adrenocortical hormone.Methods 65 patients with aldosterone producing adenoma(APA),43 patients with cortisol-secreting adenoma(CSA) and 24 patients with nonfunctional adrenal adenoma(NAA) were enrolled.They were given oral glucose tolerance test(OGTT) and then the insulin sensitive index(ISI),HOMA insulin resistance index(HOMA-IR),area under the curve of glucose(AUCG) and insulin(AUCI) were calculated.Their levels of plasma and urinary aldosterone and cortisol,plasma ACTH,plasma renin activity(PRA),plasma,and plasma potassium were also measured.The results were compared with 53 patients with essential hypertension(EH) and 40 healthy subjects.Results ① The levels of plasma and urinary aldosterone in patients of APA group were higher than those of the other groups,but the levels of plasma potassium,basic and upright PRA were lower(P 0.05).The levels of 8: 00,24: 00 plasma cortisol and urinary cortisol in patients of CSA group were higher than those of the other groups,but the level of 8: 00 plasma ACTH was lower.② The prevalence of hyperglycemia was 44.6%(29 /65) in APA group,and 48.8%(21 /43) in CSA group,which were significantly higher than 25.0%(6 /24) in NAA group,24.5%(13 /53) in EH group and 17.5%(7 / 40) in the control group(P 0.05).The values of AUCG,AUCI and HOMA-IR of the patients in APA and CSA groups also showed significant differences from those of other 3 groups(P 0.05).③ In patients of APA group the levels of 2 hPPG and AUCG 8: 00 were correlated with plasma aldosterone positively(r = 0.250,0.289,P 0.05),and with plasma potassium negatively(r =-0.301,-0.304,P 0.05).In patients of CSA group the level of 2 hPPG was positively correlated with 8: 00 plasma cortisol(r = 0.303,P 0.05);and the levels of FPG,2 hPPG and AUCG were positively correlated with urinary cortisol(r = 0.403,0.312,0.379,P 0.05).④According to the results of OGTT,all the APA and CSA patients were subdivided as hyperglycemia group and non-hyperglycemia group.The APA patients with hyperglycemia had higher level of plasma aldosterone than those without hyperglycemia.The CSA patients with hyperglycemia had higher level of 8: 00 plasma cortisol and lower level of plasma ACTH than those without hyperglycemia(P 0.05).Conclusion The patients with functional adrenocortical adenoma have a significantly high prevalence of hyperglycemia,and it is related with higher secretion of adrenal cortex hormone." @default.
