- W2377812101 abstract "Objective To observe the oxidative stress acutely induced by intravenous iron and its prevention by intravenous ascorbic acid with different doses in chronic renal failure patients and its mechanism.Methods Ten cases of chronic renal failure patients without dialysis were in the present study,whose levels for their serum ferritin were all below 300 ng/ml.The underlying diseases included chronic glomerulonephritis (5 cases),diabetic nephropathy (3 cases) and hypertensive nephropathy (2 cases).Each of them received simple iron dextron therapy (100mg),with a low dose (1.0 g) or high dose (3.0 g) ascorbic acid just before iron infusion respectively.The interval between different clinical intervention groups was 3~5 days.To evaluate the preventive effect of ascorbic acid,the levels of malon-dialdehyde (MDA),glutathione (GSH),superoxide dismutase (SOD) activity were measured before blood sample obtained and in 1,2,3 hours after the administration of intravenous iron.Results The serum level for creatinine of the patients is (7.98±2.22)mg/dl,for serum urea nitrogen,(79.43±21.22)mg/dl,for serum ferritin,(129.03±55.16)ng/ml,and for serum iron,(56.20±25.71)μg/dl.After the infusion of iron,the plasma MDA level increased significantly and reached peak level after 1 hour (4.75±1.58 nmol/ml vs 11.72±2.09 nmol/ml, P 0.001).The level of plasma SOD decreased after 1 hour 〔(52.89±3.39)NU/ml〕 and 3 hour 〔(52.49±3.84) NU/ml〕 compared to the baseline 〔(54.19±3.41)NU/ml〕,but not significantly.The plasma GSH level decreased and reached the bottom after 1 hour 〔(197.36±34.47)mg/L〕,but not significantly compared to the baseline 〔(223.94±40.79)mg/L)〕.Low dose intravenous infusion of ascorbic acid before infusion of iron dextrin could significantly decreased the peak level of plasma MDA induced by intravenous iron ( P 0.001).The plasma activity of SOD showed no obvious change with low dose ascorbic acid just before the iron infusion.It could increase the plasma GSH but not significantly.High dose intravenous infusion of ascorbic acid before infusion of iron dextrin could also decrease the peak level of plasma MDA ( P 0.001) but not significant compared with low dose group.This intervention could increase the plasma SOD activity at the time just before intravenous iron 〔(61.45±4.74)NU/ml〕 and 3 hours after the iron infusion 〔(56.62±4.04)NU/ml〕 significantly( P 0.05) compared with the simple iron infusion group (54.19±3.41 NU/ml,52.49±3.84 NU/ml).Plasma GSH went a little higher with this intervention but not significant compared with simple iron group,and there was no significant change compared with low dose group( P 0.05).Conclusion Intravenous iron commonly used to treat anemia in chronic renal failure patients might induce acute aggravation of oxidative stress in certain degree,which might be alleviated by intravenous ascorbic acid before iron infusion.High dose of ascorbic acid may have better effect than low dose." @default.
