- W2380756959 abstract "AIM To compare the effects of fentanyl and different dosages of sufentanil or remifentanil combined with propofol used in general anesthesia induction on cardiac function.METHODS Fentanyl,sufentanil or remifentanil,midazolam,propofol and rocuronium were used in induction of anesthesia.One hundred and five patients undergoing elective surgery and requiring general anesthesia were randomized to 7 groups,15 in each group.The patients in the group F received fentanyl 3 μg.kg-1,while sufentanil 0.2,0.3 and 0.4 μg.kg-1 in the group S1,S2,S3,and remifentanil 1,1.5,2 μg.kg-1 in the group R1,R2,R3.Mean arterial blood pressure(MAP),heart rate(HR),stroke volume(SV),cardiac output(CO),cardiac index(CI) and acceleration index(ACI) were recorded at 1 min before induction,1 min before intubation and 1,2,3,5 min after intubation.RESULTS No patient suffered from muscle rigidity or disturbance of ventilation.There were no statistical differences in HR,SV,CO,CI and ACI at different time-points within group F(P 0.05).MAP reduced in all the groups except group R1 after induction of general anesthesia and increased to the basal level after endotracheal intubation in the group F,S1,S2 and S3,but MAP remained low after intubation in the group R2 and R3(P 0.05).After induction,HR in the group S2 and S3 decreased significantly(P 0.05) and increased to the basal level after endotracheal intubation.MAP in the group R3 was lower than that in the group F,while CO,CI and ACI in the group R1 and R2 were lower than those in the group F(P 0.05).At 1min and 2min after intubation,CO and CI in the group R1 and R2 as well as ACI in the group R1,R2 and R3 were lower than those in the group F significantly(P 0.05).CONCLUSION Sufentanil combined with propofol in general anesthesia induction doesn’ t have a suppressing effect on cardiac function at the dose of 0.2-0.4 μg.kg-1.However,remifentanil combined with propofol in general anesthesia induction could aggravate cardiac depression and the reduction of blood pressure induced by propofol.As a result,large dose and rapid injection of remifentanil should be avoided." @default.
