- W2391366287 abstract "Objective To study the clinical values of furosemide trial in predicting hematoma enlargement in patients with hypertensive cerebral hemorrhage.Methods Four hundred fifty-one patients with hypertensive cerebral hemorrhage were diagnosed definitely by computerized tomography(CT) within 3 hours after onset of symptoms and repeated at 24 hours in order to find out the enlargement of hematomas.Blood pressures were measured in all patients before and 30 min after the furosemide injection.the changes of mean arterial pressure(MAP) were calculated before and after the furosemide injections.The relationship between hematoma enlargement and variation of MAP was investigated after the furosemide trial.Results ①In 87 cases the hematomas were enlarged and in 364 cases were not.The more decrease of MAP,the less incidence of early hematoma enlargement in cerebral hemorrhage after the furosemide trial(χ2=54.84,P0.001),the reduction value of MAP was negatively correlated with the incidence of hematoma enlargement(r=-0.94,P0.001).②After the furosemide infection,the MAP reduced to 10 mm Hg in 48.1% of patients(217/451),in which 82.8%(72/87) in the hematoma enlargement group,and 39.8%(145/364) in the non-hematoma enlargement group.There were significant differences between the 2 groups(χ2=76.18,P0.001).③Taking the decreased of MAP 10 mm Hg as positive(predicting hematoma enlargement),or taking the decreased of MAP ≥10 mm Hg as negative(predicting no hematoma enlargement),the total coincidence rate and the comprehensive evaluation index of the two predicting methods were the same as Youden index.The sensitivity of the in predicting hematoma enlargement was higher(82.8%)and the false negative was lower(17.2%).The positive likelihood ratio was 3.39.Conclusion Decrease of MAP 10 mm Hg in patients with hypertensive cerebral hemorrhage will predicate hematoma enlargement after the furosemide trial,and decrease of MAP ≥10 mm Hg,will not develop hematoma enlargement." @default.
