F.tularensis typeA causesseveral different disease syndromes depending on the methodofexposure.Ulceroglandular infections arethemost common type(ca.80%)andare characterized byfever, swollen lymphnodes, andulceration atthesiteofskin penetration, usually through thebiteofan infected tick, mosquito, or deerfly; ingestion ofF.tularensis-contaminatedfoodorwaterproduces oropharyngeal and/or gastrointestinal tularemia; andinhalation ofaerosolized bacteria results inthepleuropulmonary form.Regardless oftheroute ofinoculation, bacteria usually invade theregional lymph nodesandaredisseminated viathesystemic circulation to other organs. F.tularensis bv.palaearctica (Jellison typeB)ismore widely distributed innatureandisfoundinEurope, Asia, andNorthAmerica. Itisconsidered tobelesspathogenic thanF.tularensis bv.tularensis, although studies have identified this biovar as a cause ofdisease inFinland (14), Japan(7), andtheUnited States (6). Although F.tularensis can beisolated fromblood(8,13), diagnosis bythismethodisnotoriously unreliable. In a recentstudy inthesouthwest central United States, bacteria were isolated fromonly13.2%oflaboratory-confirmed cases (13).
