- W2401683582 abstract "Although my discussion will digress somewhat from the topic of pyelonephritis, the studies to be reported actually originated from an interest in pyelonephritis. During our early studies of experimental and human pyelonephritis, it became apparent that little was known about immune medlanisms which protect the urinary tract from invasion by gram-negative bacilli. In actuality, the evidence which is available suggests that striking differences may exist between the protective effect of type-specific antibodies to gram-negative bacilli in renal infections in experimental animals and man. Studies of pyelonephritis in animals have demonstrated disappearance of the infecting organism concomitant with the appearance of type-specific antibodies (7). In contrast, persistence of renal infection, and even the appearance of new infections, in man in the presence of high titers of O-specific antibody has been repeatedly observed (10, 11). Indeed, despite the readily demonstrable protective activity of O-specific antibody in experimental animals, there is no evidence that O-specific antibody protects against human infections caused by Enterobacteriaceae other than Salmonella. This apparent lack of correlation between the protective effects of antibody in experimental animals and man indicated the need for a critical evaluation of immune mechanisms against gram-negative bacillary infections in man. Since the vagaries of pyelonephritis make it difficult to accurately assess the role of humoral immunity in this disease, the studies to be described evaluate the effect of antibody present at the onset of bacteremia in ameliorating the severity of bacteremia caused by gram-negative bacilli. Concomitant with the onset of these clinical studies, investigations by several investigators made it apparent that gram-negative bacilli possessed a number of shared, cross-reactive antigens.
