The United States will need an additional 3.5 million well-trained providers to care for older adults by 2030 (Institute of Medicine, 2008). What will attract individuals to these professions? I interviewed six providers to find out what drew them to their work. I asked the following providers, who represent medicine, acute- and long-term-care nursing, psychiatry, social work, and homecare, what attracted them to older adults, the joys and challenges of their professions, and why they stay. Across the board, they were passionate about working with older adults and passionate (with evangelical zeal, as one put it) about promoting their profession to others. They spoke of the privilege of being part of elders' lives, the hardships of their work, and the intellectually demanding and exciting nature of the field. Irene Fleshner, an RN for thirty years, was a hospital administrator and the first chief nursing officer of Genesis Health Care, a long-term-care provider in Kennett Square, Pennsylvania. When her long-term-care nursing career began, she had no intention of staying in the field. Find out why she did. ? What shocked you about long-term care? I entered long-term-care nursing fifteen years ago. What I found were very sick people with complicated, chronic medical conditions who were, in some cases, not receiving adequate professional nursing care in institutions. There weren't enough registered nurses (RNs) and those who were there were underappreciated and, frankly, lacked the necessary skills and competencies. It was eye-opening and sad, and I thought it was wrong. We needed knowledgeable, skilled, competent professionals to care for people with complex, chronic conditions. It became my passion and life goal to change that. ? Is long-term-care nursing practiced differently than acute-care nursing? In acute care, RNs are responsible for assessing their patients, making judgments, and developing and implementing care plans based upon those assessments- tasks only RNs should do. A predominate licensed practical nurse (LPN) workforce, often the case in nursing homes, doesn't give you that ability. When I study skilled nursing homes, I see people doing tasks, lots of activity, and hard work, but little professional nursing practice. Direct-care workers do wonderful work, helping residents with activities of daily living. However, often there's no layer above them with the education and skills to direct care or implement professional nursing practices. Somehow, it was never instituted. ? Why did having so few RNs in long-term care become the norm? When I entered the field, nursing homes were the primary long-term-care setting, staffed mostly by LPNs. They weren't welcoming places for RNs, were punitive and hierarchical, and didn't provide challenges or status. There was no reason for RNs to work there. In most cases, RNs and LPNs were responsible for the same duties, with little role differentiation between the two. ? What are the biggest challenges to building long-term-care nursing?
