- W2402920523 abstract "Genetic causes for inter-individual variability response to opioids are clinical difficulties for treatment efficiency. The aim of the present study was to investigate the possible association of opioid treatment outcome with single nucleotide polymorphisms (SNPs) in the mμ opioid receptor (OPRM1) and catechol-o-methyltransferase (COMT) genes, in Tunisian cancer pain patients. We genotyped one hundred and twenty-nine cancer patients treated with different doses of morphine for 3 SNPs in OPRM1 gene (rs17174629, rs1799972 and rs1799971) and one in the COMT gene (rs4680). Associations between dose (continuous), dose escalation (yes/no) and SNP or haplotypes were investigated. Unlike other studies on Caucasian and Chinese populations, no significant association were found between the 4 polymorphisms screened and the dose of morphine needed for pain relief. This result can be explained by the genetic heterogeneity and cosmopolitan areas of our Tunisian patients compared to the others homogenous population. Les causes génétiques de la variabilité interindividuelle à la réponse aux opioïdes sont en partie à l’origine des difficultés cliniques d’utilisation de ces traitements antalgiques. L’objectif de ces travaux est d’étudier l’association entre l’efficacité du traitement opioïde et les polymorphismes (SNP) au niveau des gènes du récepteur opioïde mμ (OPRM1) et de l’enzyme catechol-o-methyltransférase (COMT) chez des patients tunisiens souffrants de douleurs liées au cancer. Nous avons effectué le génotypage de cent vingt-neuf patients atteints de cancer, traités par différentes doses de morphines, pour 3 SNP au niveau du gène OPRM1 (rs17174629, rs1799972 et rs1799971) et un au niveau du gène COMT (rs4680). Les associations entre la dose (en continu), l’augmentation de la dose (oui/non) et SNP ou haplotypes ont été étudiées. Contrairement à d’autres études effectuées sur des populations caucasienne et chinoise, aucune association significative n’a été observée entre les 4 polymorphismes dépistés et la dose de morphine nécessaire pour soulager la douleur. Ces divergences de résultats peuvent être expliquées par l’hétérogénéité génétique et les zones cosmopolites de nos patients tunisiens par rapport à des populations homogènes." @default.
