• W2404609610 abstract "In this study, we aimed to evaluate the diagnostic value of Technetium-99m methoxyisobutylisonitrile (Tc-99m MIBI) gated myocardial perfusion SPECT (MPS) in the detection of coronary artery disease (CAD) and silent myocardial ischemia (SMI) in patients with asymptomatic type 2 diabetes mellitus (DM).For this purpose, 35 patients with type 2 DM and 15 volunteers with no cardiac symptoms (control group) were included in this study. Exercise tolerance tests (ETT), echocardiography and Tc-99m MIBI gated MPS were performed in patients and volunteers. Computed tomography coronary angiography (CTCA) was performed in patients with coronary ischemia or infarct detected by Tc-99m MIBI gated MPS. The results were analyzed and compared visually and statistically.The present study revealed a high rate of silent myocardial ischemia (25.71%, N=9) in 35 patients with type 2 DM. Severe CAD in CTCA was detected in four of nine patients with ischemia or infarct by Tc-99m MIBI gated MPS (44.4%). Left ventricular diastolic dysfunction, ischemic pattern and high risk of CAD were detected in the same four patients by echocardiography, ETT and biochemical analysis, respectively. At the end of the statistical evaluation, we found that Tc-99m MIBI gated MPS showed significant correlations with CTCA, echocardiography, ETT, Hba1c level, risk of CAD and diabetic age in diabetic patients with CAD.We propose that Tc-99m MIBI gated MPS is a reliable and non-invasive method that can be used to detect silent myocardial ischemia and CAD in patients with type 2 DM.Bu çalışmada tip 2 diabetes mellitus’lu (DM) asemptomatik hastalarda koroner damar hastalığı ve sessiz myokardiyal iskeminin tespitinde Tc-99m MIBI myokard SPECT’in tanı değerini araştırmayı amaçladık.Tip 2 DM’lu olduğu bilinen 35 hasta ve hiçbir kardiyak şikayeti olmayan 15 gönüllü kişi (kontrol grubu olarak) çalışmaya dahil edildi. Hastalar ve gönüllülere Tc-99m MIBI miyokard perfüzyon SPECT, ekokardiyografi ve stres egzersiz tolerans testi (ETT) uygulandı. Tc-99m MIBI miyokard perfüzyon SPECT de iskemi veya infarkt tespit edilen hastalara BT (bilgisayarlı tomografi) koroner anjiyografi yapıldı. Hastaların ve kontrol grubunun hepsinin koroner arter hastalığı riski yönünden biyokimyasal olarak lipid profilleri incelendi. Sonuçlar hem görsel hem de istatistiksel olarak değerlendirildi.Bu değerlendirmeler sonucunda, çalışmaya dahil edilen 35 tip 2 DM’lu hastada yüksek oranda (%25.71) sessiz miyokardiyal iskemi saptandı. Sintigrafide iskemi veya infarkt tespit ettiğimiz 9 hastanın 4’ünde BT koroner anjiyografide ciddi koroner damar hastalığı bulguları saptandı (%44.4). BT koroner anjiyografide koroner damar hastalığı tespit edilen 4 hastanın hepsinde ekokardiyografilerinde sol ventrikül diyastolik disfonksiyonu, ETT’inde iskemik patern ve biyokimyasal analizlerinde koroner arter hastalığı yönünden yüksek risk tespit edildi. Yaptığımız istatistiki değerlendirmelerde Tc-99m MIBI miyokard perfüzyon SPECT, BT koroner anjiyografi, ekokardiyografi, ETT, Hba1c, koroner arter hastalığı riski ve diyabet süresi arasında anlamlı korelasyonlar bulundu.Tc-99m MIBI miyokard perfüzyon SPECT’in tip 2 DM’lu hastalarda koroner kalp hastalığını ve sessiz miyokardiyal iskemiyi tespit etmede kullanılabilecek güvenilir ve non-invaziv bir yöntem olduğu sonucuna vardık." @default.
