- W2406853544 abstract "It is a very exciting and ever changing time in the world of total joint arthroplasty, and that is only made clearer when one reviews the material included in this special American Association of Hip and Knee Surgeons (AAHKS) Supplement to The Journal of Arthroplasty (JOA). The manuscripts included were selected from the podium presentations at the 2008 AAHKS Annual Meeting in Dallas, Texas, by the Education Committee. All presenters from the meeting were invited to submit a manuscript to be considered for publication in the Supplement. These manuscripts were reviewed and edited by members of the JOA Editorial Board and an invited panel of Guest Reviewer's from the AAHKS membership. Every arthroplasty surgeon faces new challenges and opportunity as the world in which we practice continues to evolve. The content of the Supplement truly brings this to light. We really are, in fact, no longer just “arthroplasty” surgeons. The ability to manage hip pathology with arthroscopic and nonarthroplasty techniques is a field that is exploding, and we are challenged to define indications and standards of care. We have witnessed the development and optimization of specific surgical approaches to allow for the utilization of hip preserving as well as arthroplasty interventions Some clinical issues remain relevant despite being issues that have continually challenged surgeons in our field since its inception. Despite technological advances that our forefathers would likely marvel at, we still struggle with the same issues they did. We have made great gains in the realm of pain management but still have room for improvement. We have novel pharmacologic agents for DVT prophylaxis, yet we still lack randomized, prospective data to allow us to determine the definitive agent of choice. We still are truly in the process of acquiring and interpreting data on bearing surfaces in hip and knee arthroplasties in an effort to determine idealized bearing designs and composition. Efforts are ongoing to not only identify risk factors for complications but also to idealize patient management to minimize the incidence of these complications. We still lack a consensus opinion about what we tell patients in regard to activity level after hip and knee arthroplasty, but we face an ever increasing number of patients that want to maintain a high level of activity. Like all surgeons, we still wage the battle against infection and strive to reduce the incidence and perfect the management of periprosthetic infection. It is clear that the future is bright, and great opportunity lies ahead for our field, but we will have to proceed cautiously. It is understood that our care is needed presently and in the years to follow. We must continue to collect data prospectively and identify novel modes of failure and new complications that our innovation may create. We must also continue to document that the procedures we perform are safe, cost-effective, and beneficial to our patient population. Future installments of this Supplement will likely focus on these same topics, but we can only hope that our knowledge and understanding improve and increase with each year.
