Because it was assumed that in children the strength of the ACL was superior to that of adjacent femoral and tibial insertion sites or to the physis itself, trauma to this osteoligamentous complex would preferentially result in an avulsion of the tibial spine or physeal fracture. l -4 As recently as 1983, the accepted dogma regarding ACL tears in children was that ligamentous disruption occurs only after growth plate closure.5 The literature suggests that complete ACL disruption can occur in the pediatric population. Numerous articles report complete ACL tears in skeletally immature children.6--15 Children and adolescents are increasingly participating in more dangerous sporting activities, and associated ACL injuries are becoming an increasingly common presentation. Not only has increased participation led to more injuries, but a greater recognition of these injuries by the medical community has brought their prevalence to the fore front. This enhanced recognition has been aided by increased suspicion and improved methods for diagnosing ligamentous disruptions in all age groups.16 In the past, operative treatment for ACL injuries in children was commonly postponed until after physeal closure. Although this philosophy is still appropriate in most instances, there are situations in which it is not practical. Controlling the activities of children is a difficult task, given their immaturity and the increasing pressure on athletes to excel at younger ages. Programs have been created to identify and support athletes with special talent, and the rewards for success have lowered the age of entry into specialized programs.
