- W2408812529 abstract "We have measured acid absorption in the human stomach by two methods: (1) the instillation of an organic buffer, glycine, during maximal steady state stimulation to measure that acid which might have been reabsorbed by the stomach, by subtracting from the maximal acid output the acid output without glycine instillation; (2) the absorption of acid after the instillation of isotonic HC1. Previous attempts to use glycine instillation with single histamine injections have failed because glycine stimulates release of gastrin from the antrum with an augmentation of maximal secretion produced by histamine alone. The use of pentagastrin infusion under steady state maximal acid output has obviated this difficulty. Canine experiments, with separated antrum, validated this, indicating no increase in acid secretion from the corpus during pentagastrin infusion and glycine instillation into the antrum. Both pentagastrin infusion-glycine instillation and acid instillation studies were compared in 5 patients with gastric ulcer (GU), 4 patients with duodenal ulcer (DU), and 1 patient with gastric cancer. In addition, acid instillation studies alone were performed on 10 control subjects, 2 additional GU patients, 4 DU patients, and 1 gastric cancer patient. Gastric acid absorption in patients with GU in glycine-pentagastrin experiments was 16.9 ± 2.6 (sem) mEq per hr and for acid instillation 18.4 ± 2.0 (sem) mEq per hr. These values do not differ significantly from each other and are higher than control values. Healing of 2 patients with GU did not affect previous levels of acid absorption. For DU, acid absorption was estimated at 4.1 mEq per hr by glycine and 5.4 ± 0.6 (sem) mEq per hr by acid instillation. Gastric cancer patients behaved similarly to control subjects for both methods. By using two different techniques, similar results, namely the increased permeability of the stomach to endogenous and exogenous H+ ion in GU and diminished permeability in DU, have been observed. These observations confirm the magnitude of H+ absorption determined by acid instillation studies, and may indicate differences in size and barrier function of the antrum in GU as compared with DU." @default.
