• W2410369903 abstract "This paper reports the results of 60 bilateral vasovasostomies and describes the surgical technique. Restoration of fertility is requested in about 1 of 1000 vasectomies. The operation is done using a binocular loupe. A 5 cm skin incision is made over the induration of the previously sectioned vas. After identifying the vas the severed fibrous ends are removed. Potency of the vas lumen is determined. A 2-0 nylon splint is passed into the proximal end for 2-4 cm and brought out beyond the distal end where it is fixed. This is to act as an internal splint. The 2 ends of the vas are approximated and sutured together without the sutures penetrating the lumen of the vas. The lumen of the proximal end is usually found dilated and the walls thinned requiring special care in suturing. Adjacent tissues are sutured to relieve strain on the repaired vas. The end of the nylon internal splint is brought through the skin and fixed. The scrotal incision is closed. Antibiotics are given. The scrotum is supported by plastic strips. Ambulation is not allowed for a week. The internal splint is removed after 7-10 days. Liothyrinone (25 mcg daily) is given for 1-3 months. Antiinflammatory enzymes such as chymotase, varidase, or trypsillin are given for 2 weeks. At convalescence stiz-baths and scrotal support are advised. Sex activities may be resumed after 2 weeks. Semen specimens are examined every month for 3 months. If azoospermia persists for 1 year reoperation is recommmended. Only 50 of the 60 cases had semen examinations. Of the 50, 43 were found to have viable sperm 1 month following surgery. Of the 7 remaining cases 5 were proved to be unsuccessful. The other 2 had been operated on only 6 months before the report. Of the successful cases the number of spermatozoa varied from 1 to 90 million per ml, with an average of 29 million per ml. Motility ranged from 5% to 80% with an average of 34%. Normal morphology ranged from 50% to 90% with an average of 70%. Sperm did not appear for an average of 6 weeks and reached the highest counts at 9 months. Successful results by others are quoted.
