Son pronostic serait selon plusieurs auteurs meilleur chez l’enfant. Les auteurs rapportent une observation de NMO chez un enfant de 8 ans, sans antécédents pathologiques, admis pour tétraplégie flasque et chute brutale de l’acuité visuelle. L’examen ophtalmologique montrait une neuropapillite bilatérale. L’IRM médullaire objectivait un aspect de myélite étendue avec archnoïdite bifocale. La recherche d’un facteur infectieux déclenchant révélait des IgM dirigés contre l’hépatite virale A. L’évolution sous traitement par corticoïdes a été marquée par une récupération totale de la vision, une récupération motrice et sensitive au niveau des membres supérieurs et du tronc. L’enfant a gardé une paraplégie avec des troubles sphinctériens. À travers cette observation particulière par le facteur déclenchant (virus de l’hépatite A), les auteurs font une mise au point sur ce syndrome rare avec revue générale de l’ensemble des cas pédiatriques publiés dans la littérature. Devic optic neuromyelitis is a rare clinical disease that involves severe transverse myelitis and unilateral or bilateral optic neuropathy. Its pathogenesis would be explained by demyelinization triggered by bacterial or viral infections phenomena. According to several author, prognosis would be better in children. We report a case of optic neuromyelitis in an 8-year-old child with an uneventful history who was admitted because he suffered from flaccid tetraplegia and sudden decline of the visual acuity. The ophthalmologic examination revealed bilateral neuropapillitis. The medullary MRI visualized a spreading myelitis with bifocal arachnoiditis. Search for a precipitating infectious factor showed positive IgM for viral hepatitis A. With corticosteroid treatment the child achieved total recovery of vision and recovered motor and sensorial function of the upper limbs and the trunk. Paraplegia with sphincteral disorders persisted. This case characterized by the precipitating factor (hepatitis virus A), illustrates this rare syndrome. We present a general review of the pediatric cases reported in the literature." @default.
