- W2417962080 abstract "SUMMARY “Success” or “failure” of reattachment surgery cannot be equated with the successful repair of any one of the tissue types operated upon. A replant operation is not a success simply if a fracture heals or if sensibility returns. Similarly, it is not a success if the vessel functions. These are complex composite injuries for which success must be measured only by function defined in global terms for each individual patient and function of the hand as a whole rather than by any single type of tissue repaired. For the individual patient, tissue survival may be correlated with excellent functional recovery, or the replanted part may survive only as a parasite and actually be a detriment to function. The functional results of the care of patients with amputations treated with reattachment must be assessed realistically. This evaluation is essential if we as physicians are to select carefully the proper patients for replantation and avoid the all-too-common pitfall of “the triumph of hope over reason.” An evaluation scheme for results must above all be related to the patient and such a system is proposed (see Table 1). It is a most difficult problem and surely even better systems of evaluation will evolve, as all involved in this work recognize its importance. The functional result from a reattachment effort must be compared with and related to the anticipated results from reconstruction of the remaining hand without replantation, not the normal hand. An oft quoted goal for physicians is, “To cure occasionally, to relieve often, to comfort always.” In caring for our amputation/ replantation patients we must remember, “To regain normal function occasionally, to regain good function often, to rehabilitate to maximum possible function always.”" @default.
