- W2425213043 abstract "ISEE-706 Objective: Our objective was to examine hospitalization records maintained by the Centers for Medicare & Medicaid Services (CMS) and records of enteric infections reported to the Massachusetts Department of Public Health (MADPH) with respect to demography of elderly in 1993–2002. Materials and Methods: Five enteric infections: cryptosporidiosis, giardiasis, shigellosis, campylobacteriosis, and salmonellosis were assessed through comparison of age-specific rates estimated from 1262 CMS hospitalization records and 3844 laboratory-confirmed cases reported to the MADPH in elderly. Results: Over 10 years, annual rates of both hospitalization and reporting of Salmonella and Campylobacter infections were significantly decreasing (P<0.05). Hospitalization rates for giardiasis and campylobacteriosis declined faster than reporting rates. For 4 infections the rates of reporting were consistently higher than the corresponding hospitalization rates: for each case of hospitalization there were about 3 laboratory-confirmed cases of giardiasis or campylobacteriosis and about 2 cases of salmonellosis or shigellosis. In contrast, for cryptosporidiosis the number of reported cases (38) was even less than the number of hospitalizations (45). Both reporting and hospitalization rates were the highest in persons aged 85 and above. For all infections but cryptosporidiosis, in both males and females, the hospitalization rates were increasing with age (P<0.05). Reporting of Salmonella infections have also increased with age for both genders (P<0.05); however, for cryptosporidiosis a significant increase was observed only for men (P<0.05), and for giardiasis there was a decrease in reporting with age among women (P = 0.004). Age- and gender-specific rates of hospitalization and reporting were positively correlated, and the association was significant for cryptosporidiosis (P<0.001), and Salmonella (P = 0.023) and Campylobacter (P<0.0001) infections. Conclusions: The observed discrepancies in decadal and age-specific trends of hospitalization and reporting for enteric infections, and cryptosporidiosis in particular, are indicative of age-related testing and reporting practices, potentially incomplete testing, and/or substantial under-reporting among the elderly." @default.
