- W245996955 abstract "Publisher Summary This chapter provides a review of the therapeutic trials employing cytoxic chemotherapy for the treatment of multiple myeloma. Melphalan is the most widely used agent for the initial treatment of multiple myeloma. There is little evidence to support the superiority of any drug schedule. Drug doses producing some myelosuppression may give better results than those not producing myelosuppression. Intravenous melphalan may produce slightly higher response rates and longer survival than the oral form. This may be because of the erratic absorption of oral melphalan, which may be clinically apparent by the lack of myelosuppression. Corticosteroids have little activity when used alone and do not prolong survival. When used in combination with melphalan, they add little, if any, survival advantage when compared to melphalan alone. However, the combination may be beneficial in good risk patients. The limited published data at this time suggests that all the alkylating agents tested have similar activity in myeloma. Adriamycin and vincristine as single agents may have some activity in untreated patients although they have been studied in few patients. Cytoxan and adriamycin with BCNU (1,3-Bis (2 chloroethyl)-l-nitrosourea) are effective second line drugs in myeloma alkylator failures. Hexamethylmelamine, pyrazofurin, and the epipodophyllotoxins are being investigated in alkylator failures and may have activity. Except for pyrazofurin, there is little evidence for activity of any antimetabolites." @default.
