- W2461790882 abstract "目的．肺癌診療ガイドラインでは臨床病期IA期の非小細胞肺癌に対しては，肺葉以上の切除がグレードAの標準術式とされている．肺葉切除への耐術能を有さない場合は消極的縮小手術や定位放射線治療の対象となるが，その治療効果を比較した報告は少ない．方法．2009年1月～2015年3月に当科で治療した臨床病期IAの非小細胞肺癌症例のうち，消極的縮小手術および定位放射線治療を施行した症例を対象とした．結果．消極的縮小手術（部分切除）20例，定位放射線治療20例であった．以下，消極的/定位の順で示す．平均年齢73.9歳/78.9歳（P=0.05），ECOG PS（0：1：2）5：14：1/0：15：5（P=0.02），5年局所制御率84.4%/77.8%（P=0.82），5年無再発生存率73.2%/45.7%（P=0.14）であった．結論．肺葉以上の切除が困難な臨床病期IA非小細胞肺癌において，消極的縮小手術および定位放射線治療は一定の治療効果が期待できると考えられる．しかし，どちらを標準治療とすべきかの決定にはランダム化比較試験が必要である．" @default.
